Team dedicated to Working for the Doctors and the Patients at the Surgery!

Hello, my name is Rachel; I lead the clinical pharmacist team for the local Primary Care Network covering six practices in Malmesbury, Sherston, Royal Wootton Bassett, Purton and Cricklade. Having a pharmacist working directly for and within a surgery environment, so no connection with the local pharmacies, is a relatively new concept, but a much needed one!  Since taking on the role the team has now expanded and now we have two additional pharmacists, Andreea and Simble with another new pharmacist and a pharmacy technician joining us by the end of the year.

Malmesbury is our biggest practice so between the team we provide almost full time cover Monday through Friday. Due to current restrictions much of our work is being done working from home or via telephone.

The team’s main aim is to support the GPs and patients to ensure that their medication is managed in the best possible way, and help make changes when better alternatives become available. We undertake medication reviews also support the management of patients with long-term conditions. We are heavily involved with medicines safety and work with local community pharmacies and care homes.

I personally specialise in working with chronic pain, particularly with an interest in fibromyalgia and work together with patients to reduce opioid usage as recommended by the latest NICE guidelines. We support patient’s with resources and a monthly group meeting for people to share their experiences and develop alternative strategies/treatments to manage their chronic pain condition.

Thanks
