You have been invited for an annual physical health check with the surgery. NHS England has a national ambition to improve and increase the uptake of annual physical health checks for people living with a severe mental illness (SMI), especially those diagnosed with schizophrenia, psychosis, or bipolar disorder.
The check will include a physical health check with one of our health care assistants, including a blood test, blood pressure, height and weight. This will be approximately 30 minutes long.
We will also ask you some questions about lifestyle.
We believe that your mental health is just as important as your physical health, and sometimes they go together. We would therefore also like to invite you for a wellbeing check alongside your physical health check a member of our Living Well Team. This is an opportunity to discuss how your overall wellbeing and you can request this as a face to face after your physical health check, either on the same day or another day, or if you’d prefer, a telephone consultation.
The Living Well Team can help support you in quitting smoking, alcohol reduction and weight loss, as well as signpost you to other services in the area.
